We have sought to provide you an easy to use version of your Charge Conference reports.  

This year the forms are in Word (Form Mode) and Excel.  Simply click on the form to the left to open and save to your computer. Use your tab key to fill in the necessary text. Also, be sure to print a copy of the form for record keeping.
